 Written Referrals from your GP or Specialist are required for Respiratory or Sleep consultations or Diagnostic Sleep Studies, CPAP consultations, or Lung Function Tests. 

Referral forms are to be filled out by your doctor and given to you when you come into the clinic or your doctor can email or fax it directly to us. Once we received your referral, our physicians will triage it and we will contact you to make a suitable appointment. GP referrals are valid for 12 months and specialist referrals 3 months from first date of service.

Please bring along a list of your medications, any relevant X-rays or CT scans of your chest, and the results of any past breathing tests or sleep studies. Sleep Study Referrals

It is a Medicare requirement that the patient needs to have (STOP BANG>3 and ESS>8) or (OSA 50>5 and ESS>8) to qualify for rebated sleep study without physician consultation.

Please fill up and send the online pre-screening questionnaire here to receive your Medicare rebate for your sleep study. You can also print the pdf form below and bring it together with your referral.
